Research Findings on Tesamorelin and Adipose Tissue Quality in HIV
In a Phase 3 clinical analysis conducted by Lake et al. (2021), researchers investigated how the synthetic peptide tesamorelin, a growth hormone-releasing hormone (GHRH) analogue, influences adipose tissue quality in adults living with HIV who had central adiposity. Tesamorelin has been studied extensively in the context of HIV-associated lipodystrophy, particularly abnormal visceral fat accumulation in the abdominal region.
Previous clinical trials had demonstrated reductions in visceral adipose tissue (VAT) following tesamorelin administration. The Lake et al. analysis focused on a related question: whether tesamorelin also influences adipose tissue density and quality, independently of changes in total fat volume.
Methods
According to the methodology described by Lake and colleagues, the analysis used pooled data from two completed randomized, placebo-controlled trials. Participants were adults between 18 and 65 years of age living with HIV, receiving stable antiretroviral therapy, and demonstrating evidence of central adiposity.
Participants were randomized to receive either 2 mg of tesamorelin or placebo daily for 26 weeks. Researchers defined a response as at least an 8% reduction in visceral adipose tissue area measured by CT imaging, with approximately 70% of tesamorelin-treated participants meeting this criterion.
For this analysis, researchers compared participants who responded to tesamorelin with participants in the placebo group. CT scans were evaluated by a blinded reader to measure both visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) density in Hounsfield units (HU).
Higher adipose tissue density on CT imaging is generally associated with smaller adipocytes and has been investigated as one potential marker of adipose tissue quality.
Findings
Lake et al. (2021) reported that after 26 weeks, participants receiving tesamorelin demonstrated statistically significant increases in both VAT and SAT density compared with placebo.
Mean VAT density increased by 6.2 HU in the tesamorelin group compared with 0.3 HU in the placebo group (p < 0.0001). SAT density increased by 4.0 HU compared with 0.3 HU in the placebo group (p < 0.0001).
Importantly, these changes remained significant after researchers adjusted for baseline fat density and adipose tissue area. This suggested that the observed changes in tissue density were not explained solely by reductions in overall fat quantity.
The researchers also explored relationships between adipose tissue density and metabolic biomarkers, including measures related to insulin resistance and cholesterol. Associations varied depending on the biomarker examined, highlighting the need for further investigation into the relationship between growth hormone signaling, adipose tissue biology, and metabolic regulation.
Conclusion
The analysis by Lake and colleagues expands the scientific understanding of tesamorelin by suggesting that its effects on adipose tissue may involve more than changes in visceral fat volume alone.
In participants who responded to tesamorelin, changes in both visceral and subcutaneous adipose tissue density were observed, providing additional insight into how GHRH-related signaling may influence adipose tissue characteristics.
Further research is needed to determine the physiological significance of these density changes and how they relate to long-term metabolic and cardiometabolic outcomes.Important Notice
